To Whom to Report: Title IX Coordinator
What to Report (Responsible Employees should use the Complaint Form):
  • Your name, position, and contact information
  • Date, time, and location of alleged incident
  • Date the alleged incident was reported
  • Full name and contact information of the respondent (if known)
  • A brief description of the incident
  • Names of witnesses, if available
  • Whether the complainant has requested anonymity
  • Whether the complainant has reported the alleged incident to someone else
Confidentiality:
  • You should not share the information to anyone other than the Title IX Coordinator.
  • You should not share information with law enforcement without the complainant's consent.
  • You may want to inform the complainant that information may be shared with investigators, witnesses, the respondent, and with other college officials whose job responsibilities involve providing interim measures or accommodations.
  • You can also inform the complainant that personal information will be shared when required to be disclosed by law.

Inform the complainant of the available support services, which should be included in the College's policy when you believe that immediate attention is required. Otherwise, it is appropriate to refer questions to the Title IX Coordinator.

Talking points when someone reports a possible violation of the Title IX Policy:
Before you go any further, you should know that I am an employee who is required by law to report any incidents that may violate our Title IX Policy. If you want to discuss this with someone who does not have this obligation, please consult the College's resource list. If you want to report the incident to police, contact campus police (or campus security, or the Title IX Coordinator will be able to assist you). If the incident occurred off-campus, you can also contact the local police.
What not to do:
  • Do not try to investigate
  • Do not promise anonymity or make statements about what might happen
  • Do not discuss the matter with anyone else except the Title IX Coordinator
